Elite Fitness of Huntington

I, Member, acknowledge that I am voluntarily participating in physical exercise activities and using the facilities and equipment at Elite Fitness of Huntington, located at 11 Stewart Ave, Huntington, NY 11743.

ASSUMPTION OF RISK I understand that physical exercise involves inherent risks including but not limited to: physical injury, muscle strains, sprains, fractures, heart attack, stroke, or even death. I voluntarily assume all risks associated with my participation in any and all activities at this facility.

RELEASE AND WAIVER I hereby release, waive, discharge, and covenant not to sue Elite Fitness of Huntington, its owners, officers, employees, trainers, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury that may be sustained by me while participating in any activities at, or in connection with, Elite Fitness of Huntington.

MEDICAL ACKNOWLEDGMENT I affirm that I am physically fit and have no medical condition that would prevent my full participation in exercise activities. I agree to notify Elite Fitness of Huntington of any changes to my health status.

RULES AND REGULATIONS I agree to abide by all rules and regulations of Elite Fitness of Huntington. I understand that failure to comply may result in the termination of my membership without refund.

PHOTO/VIDEO CONSENT I grant Elite Fitness of Huntington permission to use my photograph or likeness in marketing materials, social media, and promotional content.

I HAVE READ THIS WAIVER AND RELEASE, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT. I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
